Patient Retention Professional

This role plays an important part in strengthening patient relationships and continuity of care during periods of organizational change. You will support retention initiatives related to provider departures, center relocations, mergers, acquisitions, and other transitional situations. The position combines program coordination, project management, patient and staff communications, outreach readiness, training support, and data reporting. You will work cross-functionally with operational, call center, and other teams to ensure outreach programs are executed accurately and on schedule. Your attention to detail and ability to coordinate multiple workstreams will directly contribute to positive patient experiences and retention outcomes.
