We are looking for an experienced Nurse with relevant clinical expertise and a passion, drive, and commitment to develop and deliver community Stoma Care services. This role involves collaborating with our partners and supporting patients in the Sunderland & South Tyneside (North East) areas.

We are looking for a Nurse who can work as an independent practitioner who is self-motivated and has a winning mindset. They need to be a strategic thinker who wants to build on their commercial awareness and skill and who has the confidence and courage to challenge.

The Role

• Deliver professional and ethical care to patients

• Support the patient pathway from hospital to home, providing home visits and community clinics

• Effectively and efficiently plan, manage, and organise caseload and time

• Communicate effectively and provide feedback to patients / carers and referring Healthcare professional

• Undertake an Appliance Use Review (AUR) where appropriate as per company and DoH Guidelines

• Maintain clinical records in accordance with Company, Legal and Professional requirements

• Instigate working practices to ensure collaborative working between different stakeholders e.g., acute and community health care teams

• Generate and maintain effective working relationships with health care professionals across the Region

• Deliver educational support to fellow health care professionals

• Undertake service audit/evaluation activities

The Requirements

• Registered General Nurse
• Nursing Degree Qualified desirable
• Expert clinician in Stoma management
• Sound interpersonal and relationship building skills
• Teaching experience
• IT proficiency
